Tip of the trial needle is advanced to the target structure under constant visual guidance while avoiding scratching of the cartilage on the medial femoral condyle. If the needle itself cannot reach the target structure, it is assumed that bulkier operating instruments cannot be advanced and therefore the instrument portal needs repositioning. Multiple needle insertions maybe needed before obtaining the correct portal insertion but this is acceptable as the resultant damage is far less than that due to an unfavorably positioned instrument portal.
