Fluid extravasation

If the capsular incision is too large while the subcutaneous and skin incisions are too small, irrigation fluid will dissect into the subcutaneous tissue. This expands the subcutaneous layer and shifts the tissue layers creating an obstacle to instrument insertion. The remedy is to slightly extend the cutaneous and subcutaneous incisions.
