Creation of posterolateral portal using high anteromedial portal

The arthroscope is first introduced through the high anterolateral portal. A switching rod is introduced through the high anteromedial portal and the knee is moved into figure of four position . The switching rod is passed in front of acl into the posterolateral recess. The arthroscope is removed from the antero lateral portal and the sheath is introduced over the switching rod into the posterolateral recess which is then inspected.
